Below you will find the contact information to reach our Utilization Department (UM) for any referral or authorization questions or request copies of criteria used in decision making.

UM, Staffs are available from 8:00 am -5:00 pm Monday through Friday to answer any questions related to the authorization and referral processes.

An after-Hour nurse is available after 5:00 pm Monday through Friday, Weekends, and Holidays for Urgent issues. You may also request a copy of the criteria used in the decision making process by calling our toll free number 1(855) 744-3395 / TTY: 711 for Relay service. UM, Staffs are bilingual in Spanish, or we can help set up an interpreter for you.”
